Insurance & Rates

Please be aware that, I no longer accept assignment for Medicare Part B as an ‘in-network’ provider. I am considered ‘out-of-network’ for ALL insurances, private and otherwise. You can call Medicare and ask for out of network approval for your sessions. Depending on your Diagnosis and treatment Medicare will determine if you are eligible for reimbursement. Please call Medicare for clarification and  the necessary paper work to fill out for possible out of network  approval.

 

However, if you are privately insured and choose to submit for reimbursement, I am happy to submit   a record of our sessions (and your payments) in the form of a ‘superbill’, which I will submit to your insurer as a courtesy.  Out-of-network therapy services are typically covered only in part by health insurance or employee benefit plans. Please check your coverage very carefully by asking the following questions of your insurance provider:

  • Do I have mental health insurance benefits? If so, is my outpatient mental health insurance provider the same as my general health insurance provider? (Often, your mental health insurance provider will be named on the back of your insurance card.)

  • What is my out-of-network deductible and has it been met for this calendar year?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount and co-pay/co-insurance fee per therapy session?

  • Is approval or a referral required from my primary care physician?

 

Rates

EMDR Intensive Costs:

 Services are billed at $300/hour for the initial consultation. If it is decided at that time that you are clinically appropriate for the intensive then all consecutive sessions will be billed at $250/hour.

The total cost of treatment must be paid in full at the time of the intensive. A deposit of half of the treatment hours is due at the initial scheduling of the EMDR Intensive therapy to secure your appointment times.

EMDR Intensive Cancellation Policy

No Refunds of deposits will be given if the EMDR intensive is cancelled. You may be eligible to reschedule your intensive at another time. You will have to discuss this with the provider.

I accept cash, check, Zelle, debit cards (including HSA/FlexSpend debit cards), Visa, Mastercard, American Express, and Discover credit cards. Please be aware that there is a 3.5% credit card fee added to the deposit or session paid.
